All consultations begin through the intake form. Complete the short questionnaire to describe your case, select a package, and submit your booking request. Dr. Taslidere personally replies within 24–48 hours with available times and a payment link.

What to prepare

  • A recent panoramic X-ray (OPG) — taken within the last 12 months
  • A CBCT scan if you have one (necessary for implant cases)
  • Photographs of your teeth (front-facing smile, side profile, upper and lower arches if available)
  • The full written treatment plan from the clinic, including itemised procedures and total cost
  • Any previous dental records that may be relevant (existing crowns, previous implants, etc.)

You can submit these after completing the intake form — they are not required at the time of booking.
